Transfer a Patient

Blanchard Valley Health System is prepared to help care for your patients. We accept patient transfers through our seamless one-call transfer process. Simply call 1.844.BV1CALL to initiate the transfer process.

We will need the following information upon your call:

• Patient’s Name
• Date of Birth
• Alert/Oriented Status
• Patient’s Primary Care Provider
• Patient’s Problem/Diagnosis & Pertinent Test Results
• Transferring Provider’s Name
• Transferring Facility
• Contact Number

Following your call, we will work to secure the appropriate providers to accept the patient and return a call to you. Transfers will be accepted as provider coverage allows. Transportation for the patient may be arranged through HANCO EMS if needed.
