Transforming Episode Accountability Model (TEAM)

At Blanchard Valley Hospital, we are committed to improving the patient experience before, during, and after surgery. The Transforming Episode Accountability Model (TEAM) is a Medicare initiative designed to support high-quality, coordinated care throughout your surgical journey—from the operating room through recovery.

TEAM helps hospitals and healthcare providers work together to reduce avoidable hospital readmissions and improve overall outcomes for Medicare patients.


Surgical Procedures Included in TEAM

TEAM currently applies to five specific surgical procedures:

  • Lower extremity joint replacement

  • Surgical hip/femur fracture treatment

  • Spinal fusion

  • Coronary artery bypass graft (CABG)

  • Major bowel procedures

If you receive one of these procedures at Blanchard Valley Hospital, your care will be included in the TEAM model.


Your Medicare Benefits Stay the Same

You will continue to receive all the same Medicare benefits, rights, and protections you have today.

  • Medicare will still cover all medically necessary services.

  • There is no change in the amount you pay for your care.

  • Providers will continue submitting claims to Medicare as they always have.

Because TEAM is a Medicare-required program, patients cannot opt out when receiving services at a participating hospital—such as Blanchard Valley Hospital.
However, you keep your freedom of choice and may receive care at a non-participating hospital if you prefer.


How TEAM Works With Providers

TEAM encourages hospitals and other healthcare organizations to partner more closely to support patients. These partners—such as skilled nursing facilities, home health agencies, physicians, and other healthcare professionals—are called TEAM collaborators.

  • TEAM participants may share Medicare incentive payments with these collaborators.

  • These arrangements do not change your Medicare coverage or what you owe.

  • The goal is to improve communication, streamline care, and support your recovery.

  • Patients often experience smoother transitions from hospital to home or rehabilitation.

  • There is a reduced risk of unnecessary return trips to the hospital.
