Outpatient Pharmacies

In Findlay: Center for Medication Management (CMM), 419.429.7002, is an on-site outpatient pharmacy located within Blanchard Valley Hospital and is available for your medication needs. Our Center for Medication Management can fill prescriptions Monday - Friday, 9 a.m. - 6 p.m.

Refills can be made to CMM by using the BVHS online system. An automated, after-hours prescription refilling system can also be accessed by calling 419.429.7002


In Carey: Harry's Pharmacy, 419.396.7977, is located inside the Carey Medical & Diagnostic Center. Prescriptions can be filled Monday - Friday, 9 a.m. - 6 p.m. and Saturday, 9 a.m. - 1 p.m. The prescription pick-up area is closed from 1:30 p.m. - 2 p.m. Monday through Friday.

Harry's Pharmacy offers home delivery, call-ahead curbside delivery, in-store consultations and a convenient storefront. 

Cardiac Medication Center

The Cardiac Medication Center (CMC) is located behind Blanchard Valley Hospital at 1816 Chapel Drive, Suite I, Findlay. The Center offers an expanded focus on heart-related medication management and support for overall cardiovascular health, while continuing its warfarin management to assist patients with their healthcare. Each patient works one-on-one with a clinician to develop a program based on their needs. As part of the CMC, you will receive information about your condition, learn how medication affects recovery, and have medication dosages monitored/adjusted in collaboration with your physician. After each visit to the CMC, your primary physician receives a summary report that includes current medication doses, progress notes, and laboratory test values. Patients who have questions may contact the Cardiac Medication Center at 419.423.5177.

The program currently offers the following services:

  • Fingerstick (point-of-care) INR testing for medication monitoring
  • Monitoring and adjustment of warfarin (Coumadin©) dosages

It's easy to enroll in the program

If You are an Inpatient:

Your primary care physician can write an order to enroll you in the program. The CMM pharmacist will handle the rest.

If You Have a Referral from Your Primary Care Physician:

Your physician’s office must complete the necessary documents and fax them to the CMM. A pharmacist will then contact you to arrange your first visit.

Inpatient Pharmacies

If you’re a patient at Blanchard Valley Hospital or Bluffton Hospital, our pharmacists are available 24 hours a day, seven days a week, to provide medications ordered by your physician. We participate in your care planning, discharge, and provide medication education before you leave the hospital.
