As a primary care provider, EasternWoods Family Practice offers preventative care, wellness visits, health screenings, vaccinations, guidance with medication adherence, and chronic disease management.

Our providers treat conditions including respiratory illness, diabetes, gastrointestinal and urologic conditions, dermatology, depression and anxiety, sore throats, ear infections, strains, and other injuries.

We see patients of all ages, and we offer extended hours to accommodate their schedules. Additionally, we offer care navigation to help patients manage chronic conditions such as diabetes.
